Appointments can be made by telephone or email. Please leave details of suitable days and appointment times. Clinic hours will normally be between 9am and 5pm Wednesday to Saturday, but I can be flexible if these times are unsuitable for you.

Questionnaire

Once your appointment has been made, please download the questionnaire and either type on it and email it back to me 24 hours before the appointment, or print it and bring it along with you.

Please allow at least half an hour to answer the questions, and try to answer them in as much detail as you can. This will allow us to spend the appointment time exploring your case more closely.
